A REVIEW OF RECORDS WHICH REVEALED STAFF MEMBERS MANDATED REPORTER CERTIFICATES HAVE EXPIRED.

By 8/24/23 Licensee agreed to inform all parents about the incident and obtain acknowledgement. Verification to be submitted to Licensing. Per investigation, Licensee failed to report parents unusual incident of venomous snake sighting in the premises during day care hours while children were present in the vicinity. This poses a potential risk to health and safety of children.

Per investigation, Licensee failed to report unusual incident to Licensing of venomous snake sighting in the premises during day care hours while children were present in the vicinity. This poses a potential risk to health and safety of children.

Based on record review the licensee did not comply with the section cited above. 2 staff SHANI ROSENBAUM and MELISSA MARICICH did not have fingerprint associations to the license. Director stated they attempted to send transfer requests to Licensing Office when staff were hired in 2018. She was under the impression they were associated. This poses/posed a potential health, safety or personal rights risk to persons in care.
