Administration of Child Care Licensing…(c)(1) A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph…this requirement was not met by: 1 Assistant Director stated that she shall provide a copy of the report from 12/23/2025 to all parents and have them complete the LIC 9224 form. In addition, all other files shall be reviewed to ensure the LIC 9224 form is on file for all other children in care. Copies of the LIC 9224 shall be sent the Fresno Regional Office by 1/29/2026. Based on interview and record review, LPA observed only 14 children files had completed Acknowledgement of Receipt of Licensing Reports (LIC 9224) from the type A citation issued on 12/23/2025. This poses a potential health, safety or personal rights risk to persons in care. 8

LPA Hernandez’s interviews and review of facility records it was revealed that daycare staff hit day care children and daycare staff handled day care children in a rough manner. Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1). Staff reported that on 6/5/2023, during transition from Classroom #5 to the facilty's gymnasium, Child #1 was left unsupervised in Classoom #5 for approximately 15 minutes (see LIC809 for further details). 1 Director Curbow stated that she will submit a written plan of action, detailing how the licensee will ensure adequate care and supervision is provided to children at all times, to the Fresno Community Care Licensing Office by 6/9/2023. This poses an immediate risk to the health, safety, and/or personal rights to children in care. A civil penalty of $500.00 was assessed. Director stated that she will also review supervision requirements with staff, and require staff to sign an acknowledgment confirming their understanding of regulation 101229(a)(1), and their role in providing visual observation of children, also due by 6/9/2023.
