Based on observation, the licensee did not comply with the section cited above as LPA observed in the Red Room a bottle of Lysol aresol spray, a bottle of liquid Clorox spray and a bottle of liquid all purpose cleaner in the bottle of an unlocked cabinet. In the Blue, Yellow, Red and Green Rooms, LPA observed, multiple adult size scissors, AA batteries and multiple sharpie markers in an unlocked drawer which poses an immediate health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above as LPA observed fire drill log for Blue room was conducted on 10/16/2025, Yellow Room fire drill conducted in September no date or initials documented on log, Red Room no date or initials documented on log and Pink room fire drill conducted on 10/25/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above as LPA observed in the Pink Room children's mats and sheets are stored in a cubbie where children's bedding comes into contact with other bedding. Teacher confirmed children's bedding is stored in cubbie after each nap and sheets are laundered every Monday which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above as LPA observed 5 out of 5 child files did not have an Admissions Agreement which poses a potential health, safety or personal rights risk to persons in care.
