(a) In addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met, evidenced by: Director will conduct a training with staff regarding supervision. Director will submit proof of training to LPA Tillery. Parent and staff interviews revealed that child #1 received an injury to the face, that staff did not observe, due to staff member having her back turned while changing a diaper. 8
