No child(ren) shall be left without the supervision of a teacher at any time,... Supervision shall include visual observation. This requirement is not met evidenced by: 1 By POC 10/20/2023, Licensee will submit written plan how children will be supervised at all times. Based on observation, LPA observed at 2:50PM that a child walked from the bathroom to the classroom without any staff present and at 2:54PM, a child walked from the classroom to the cubbies without any staff present. This poses an immediate risk to the health and safety to the children in care. 8

This requirement is not met as evident by: 1 Assistant Director submitted LIC 9182 with a valid ID for S-1 during inspection to the San Jose Regional office. Based on record reviews, S-1 has fingerprint clearance and is associated to infant license, which poses a potential health and safety risk to children in care. 8 Licensee will check with licensing to ensure that S-1's fingerprints were transferred.
