Based on observation the licensee did not comply with the section cited above in LPA observed hot water coming out of faucets in the restroom that children use, the floor was wet from flooding of either toilet. The floors needed to be deeply cleaned and vacuumed, basic up keep of the facility needs to be maintained.which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above, as Staff #1, who was present today, indicated that she has provided direct care and supervision to day-care children on different occasions; this individual is not fingerprint cleared through the Department as required. This poses an immediate health, safety or personal rights risk to persons in care.
Based on records review, the licensee did not comply with the section cited above as today there was not at least one person with CPR certification on site. This poses a potential health, safety or personal rights risk to persons in care.
