Based on observations, records review and interview, the licensee did not comply with the section cited above. During the staff records review, LPA observed 2 out of 5 were missing Health Screening Reports (LIC 503), which poses/posed a potential health, safety or personal rights risk to persons in care. S1 and S3 needs Health Screening Report (LIC 503).
Based on unusual incident report dated 05/30/24, & staff#1 phone interview, child#1 and child#2, pulled down their pants while playing on playhouse and child#1 pooped on playhouse planter area and staff#1 did not observe it. This poses an immediate health, safety and personal right risks to the children in care. 8 A written letter along with staff sign in sheet will be submitted to LPA by 06/20/24.
Based on today's inspection, the licensee did not comply with the section cited above LPA observed paint of two play structures are chipped and tree branches on the wood chip playground were really low in the height of a child which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on facility sign in/out sheet review for 02/13/23 the licensee did not comply with the section cited above in 3 out of 32 present children were not signed in which poses/posed a potential health, safety or personal rights risk to persons in care.

... 1 Per director, Andrea Ventura staff #1 will complete required Criminal Record Clearance by plan of correction due date and a copy of Livescan form will be submitted to LPA Eivazian. Per director, once staff#1 grant a criminal record clearance will come back to the school. On 03/14/2022 during a tour of the facility LPA Eivazian observed staff#1 who was present in Room A did not have finger print clearance in Licensing Information System (LIS). This poses an immediate health and safety risk to the children in care. $500 Civil Penalty was accessed.
Based on staff interviews on 03/14/2022, Child#1 hit child #2 on 03/03/2022 with a wooden block and child#2 sustained a small opening on the left side corner of the eye by the end of eyebrow line and this incident was not reported to licensing. This poses a potential health, safety and personal right risk to the children in care.
