. 1 Per Director, they will conduct a staff meeting to review Personal Rights and provide LPA with a copy of the meeting agenda topics and staff sign sheet by the plan of correction due date of 12/10/2024. The findings discovered during the investigation conducted by the Investigation Branch that staff used an unusual form of punishment and staff handles children in a rough manner.
Based on observation, interview, record review, the licensee did not comply with the section cited above in that the IB investigator discovered during the investigation that Staff #4 was aware that Staff #1 had a history of violating children’s personal rights and did not report all incidents to the Department.

The requirement is not met as evidence by: 1 Licensee stated that they will provide written documentation for termination for Staff #1, Staff #2, and Staff #3 and Director will provide LPA copy of the termination documentaiton by the Plan of Correction due date 07/24/2024. Based on observation,interview, record review, the licensee did not comply with the section cited above In that Personnel Records were not readily available upon request. which poses a potential health, safety or personal rights risk to persons in care. 8
