Responsibility for Providing Care and Supervision : The licensee shall provide care and supervision as necessary to meet the children's needs... Supervision shall include visual observation. This requirement was not met by evidences by... 1 Facility will provide training to staff regarding ratio and supervision. Director will provide a sign in Training sheet signed by staff attendees to the Palmdale Regional Office by email. Based on IB investigation and LPA's review C1 was injured, due to lack of supervision, at the time of the incident Staff #1 was filling the water table which was out of her sight of the children. This is an immediate risk to the health and safety of children in care. 8

According to Director, the green room sink fixture will be removed entirely and the pipe will be replaced by 8/30/22. During inspection Director covered water faucet and placed a out of service sign on the sink. Parents will be notified of lead levels with class schedules by 8/26/22 and a copy will be faxed, email, text to the Palmdale Regional Office. Per SWRCB results facility was identified as having high lead level of 22.6. Sink water in room Green is accessible and was utilized for edible science projects with children. (no posting/signs of out of service) observed. According to facility Director, as of 8/22/22 facility became aware of results.
