This requirement was not met by evidence by... 1 Facility will have staff indiviually follow up with Doctor/medical office and collect required immunization with name and date of birth on record. Facility will email a copy to the Palmdale Regional Office by due date. File review, documentation, and interview. It was disclosed six out of eight staff do not have the complete required immunization on file with full name and date of birth. According to staff medical records with immunization are being requested from Doctor's office. This poses a health and safety risk to children in care 8

Criminal Record-(b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption from disqualification, pursuant to subdivision... this requirement was not met as evidence by 1 Facility will have staff complete livescan and not return until cleared. Staff #10 was removed from the premises and went to get finger printed. Facility will email Palmdale Regional office once clearance letter has been received. Based on record review. LPAs reviewed staff files and Staff #10 is not fingerprint cleared. This poses an immediate health and safety risk to children in care. 8

This requirement was not met as evidence by: 1 Director and licensee agreed to create a plan to complete requirements to be fully qualified The facility will submit to the Department certificates and transcripts by email by due date. Based on interview, observation, and record review, facility staff are not able to provide required CDE units, record keeping orientation and preventative Health and safety certificate to be a qualified Director. Facility admitted be acting as the Assistant Director which poses is a potential Health and Safety Risk to children in care. 8
