Based on observation, the licensee did not comply with the section cited above in that facilty did not lock poisons located in bathroom cabinets which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on LPA observation, the licensee did not comply with the section cited above in that 3 trIcyles were not maintained in good working order, plastic tires were coming off of the wheel and rubber matting under play structure is ripping apart which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in that a current water lead test result could not be provided to the department which posed a potential health, safety or personal rights risk to persons in care.
Based on the observation, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During inspection of the Incidental Medical Service medications LPA observed C1 with medication store at the facility, however, C1 medication did not have the original label included in the baggie where the medication was store.
