Based on record review, the licensee did not comply with the section cited above in that 1 out of 5 infant files are missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interview with the Director and observation of her conversation on the phone with the District office, the District refused to allow the documents off site. LPAs were unable to verify immunizations, criminal record clearances, transcripst and other documents that, if absent, constitutes a potential risk to the health and safety of children in care.
Based on LPAs observation, two areas of dirt on the infant playground have small rocks and with other organic debris,and are accessible to children, constituting a potential risk to the health and safety of children in care.
