During the course of the investigation, LPA conducted interviews with director and other people involved that the inicdent that occured on 11/4/22 resulting in a child becoming lost was not reported to Licensing Department. This poses a potential health and safety risk to children in care. 8 Director will send copy to LPA by set due date, 3/10/23.
