101216 Personnel Requirements (g) All personnel, including the licensee, administrator and volunteers, shall be in good health... (2) Each person specified in (g) above shall have a health-screening report... This requirement was not met as evidence by: 1 Licensee to correct deficiency by maintaining health screen reports, which includes valid TB clearances, in staff records. Licensee to submit health screening reports for Staff 1, Staff 3, and Staff 4 and TB results for Staff 1 and Staff 3 to LPA Billones via email provided no later than 5/22/2026. Based on staff records review, the Licensee did not comply with the section cited above in 3 staff records reviewed, which poses a potential health, safety, and personal rights risks to children in care. Staff 1, Staff 3, and Staff 4 records did not have a valid health screening report. 8

Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 7 staff files reviewed, which poses an immediate health, safety or personal rights risk to persons in care. LPA notes one staff (S4) did not have a criminal record clearance nor an association to the facility. S4 was not listed on the Guardian roster as of 11/10/2025.

Based on record review, the licensee did not comply with the section cited above in 2 of 5 children's records reviewed, which poses a potential health, safety or personal rights risk to persons in care. C2 and C4 did not have a current infants needs and services plan.
