Based on observation, the licensee did not comply with the section cited above in that an EpiPen for a child in care was expired which poses a potential health, safety or personal rights risk to persons in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. 1 Facility will permanently shut down all water fountains in the building and close the angle stops, and contract with a bottled water service to provide water for all water to be consumed by children. Based on lead testing (retesting) results, 3 of 4 faucets re- tested have lead levels exceeding 5.5ppb which poses a potential Health and Safety risk to children in care. 8

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. 1 A Plan of Correction has already been issued by the facility, publically posted and issued to parents of the children in care. The plan calls for taking the affected faucets out of use and placing a "Do not use" sign on them until new faucets are ordered to replace the existing ones. Based on lead testing results, 2 of 10 faucets tested have lead levels exceeding 5.5ppb which poses poses a potential Health and Safety risk to children in care. 8

Based on record review, the licensee did not comply with the section cited above in that one staff member's personnel file was missing the signed LIC508 Criminal Record Statement.which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that two staff members files were missing the signed LIC9052 Employee Rights form which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in one child's file did not contain the signed LIC995 Parent's Rights form in the file which poses a potential health, safety or personal rights risk to persons in care.
