Based on record review provided by licensee indicating (ALE) of 7.1 ppb in outlet A-Sink Faucet-kitchen. These results indicate out of compliance over (ALE) 5.5 ppb, which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Licensee failed to ensure the personal rights of children in care to safe and healthful 1 Site Supervisor stated she will inform staff/ parents of the requirement to wear appropriate face coverings while indoors. Staff/ parents to sign an agreement of understanding. Staff Signed agreement(s) are to be submitted to CCL by 2/18/22. Parents signed agreement(s) are to be kept on child's file. accommodations, and engaged in conduct inimical to the health, welfare, and safety of children in care. On 12/8/21 LPA observed staff/ children 2 years or older, not wearing appropriate face coverings while indoors as required by the State Public Health Officer Order dated June 11, 2021 requiring compliance with CDPH Guidance for the Use of Face Coverings, and the Cal/OSHA COVID 19 Prevention Emergency Temporary Standards. Site Supervisor is to provide LPA with a written statement indicating she will make sure cots are equipped with a sheet when children use them for napping. Written statement is to be submitted to CCL by 2/18/22. On 12/8/21 LPA Iglesias observed the cots that children were napping on, were not equipped with a sheet to cover the cot. Additionally, Site Supervisor stated she was unaware of the regulation stating that cots needed to be covered by a sheet. This poses a potential risk to the health, safety, or personal rights of children.
Site supervisor disposed of the Hornet and Wasp Killer spray and securely locked the storage room on 12/8/21. Site Supervisor to submit a written statement to CCL by 2/18/22, explaining how poisons will be made inaccessible to children in care. On 12/8/21 LPA Iglesias observed a Hornet and Wasp Killer spray inside of an outside storage room. However, the door to the storage room was not properly locked, which made the spray accessible to children. This poses a potential risk to the health, safety, and personal rights of children in care. 8 This is an amended version of the original report crafted on 2/15/2022.
Based on observation the "bathroom cleaning supplies" cabinet was not locked. The cabinet is located in a restroom accessible to children. Inside the cabinet LPA observed 2 bleaches, Clorox (bowl cleaner), and Microban Sanitizing Spray. This poses a potential risk to the health, safety, or personal rights of children in care.
