Based on records review, and interviews conducted, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. On 12/27/2023 S-1 (Teacher) and S-2 (Aide) supervised 26 children in the preschool classroom. 8 AB633 Parent Notification is required. According to AB 633, all parents of children currently enrolled and any future children being enrolled for the next 12 months must be provided with this report which contains this type A deficiency.

This requirement was not met as 1 Director May will submit an email to CCLD stating what facility will do to ensure children are supervised at all times by POC date. evidenced by Staff failed to see child bite on 10/30/2023 resulting in injury and only observed the child who was bit crying and teeth marks on child's hand.
