Based on record review, Staff 1 did not have an immunization record for pertussis and measles. The licensee did not comply with the section cited above in 1 of 1 Teachers does not have current immunization record on files which poses a potential health, safety or personal rights risk to persons in care. 1 Licensee will have Teacher provide proof of immunization for Pertussis and measles on or before the plan of correction date.
The incident involves a two year old child wetting on herself and not having a change of clothing and staff had to find clothing around center to change child. Child is to have changes of clothes at center this can pose a health & safety hazard. 1 Licensee will ensure that each child has a proper change of clothing for their personal use while in care, staff will conduct proper checks to ensure this is being done.
Per Prinicipal, they will provide training on Personal Rights, complete Mandated Reporter, training LAUSD policy on child abuse and corporal punishment by the plan of correction due date 10/23/2025. The licensee did not comply with the section cited above in that Staff #1 physically moved Child 1 and Child 2 by grabbing them under their arms and pulling them across the room which poses an immediate health, safety, or personal rights risk to persons in care.
