On 9/26/2024 LPAs observed teachers and directo in each room remove cleaning solutions and place them in a cabinet with a latch making cleaning solutions inaccessible to children. LPAs observed cleaning solutions in classroom bathrooms A, B, C, and playground storage that had no safety lock and is accessible to children this poses a potential health, saftey, or personal rights risk to the children in care.

On 11/10/2021, Child #1 fell while coming 1 Licensee was advised to move the equipment to another part of the outdoor yard where there’s cushioning material. Licensee was informed that a supervising adult should always be present when children are using the equipment. Licensee will send photos of the location of the equipment on or before March 15, 2022. off the “Jump-a-roo” equipment and hit the back of head on the pavement. The jumping equipment was placed directly on fake grass/turf and did not have a cushioning material that absorbs falls. This is a Type B violation and poses a potential threat to the health and safety of children.

On 11/10/2021, facility did not operate in accordance with the terms specified in the plan of operation by failing to provide a 1 Licensee to submit a written plan on how facility will adhere and operate in accordance with the terms specified in the plan of operation. Plan of correction due by March 15, 2022. written incident report to Child’s parents. This is a Type B violation and poses an immediate risk to the health and safety of children in care.
