Based on observation, interview, record review, the licensee did not comply with the section cited above. During the 9:00 a.m. inspection, the LPA observed no background clearance for Staff 5 (S5). During the interview, the Director and S5 stated S5 have been employed since 9/2025.. which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During the inspection at 9:00 a.m., the LPA observed that Child 1 (C1) medication had expired in 3/2026,which poses a potential health, safety or personal rights risk to persons in care.

LPA observed that room 1 and room 2 have Lysol wipes and Sunscreen spray (Alba Botanica) in their classroom children's counter without the children present, which posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review. The licensee did not comply with the section cited above in. At 9:00 am, during the file review and interview, Director, LPA observed that 4 out of 5 reviewed staff files did not have current Mandated Reporter training certificates and were expired, which posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review the Director did not comply with the section cited above in The Director confirmed she did not conduct a disaster drill within the past 6 months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, The did not comply with the section cited above in 1 out of 5 staff member. Director admitted that she fogot to follow up for S2 for the current mandated reporter training certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 regulation, Director confirmed that facility has not completed Lead Testing as of 07/14/2023, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in 1 out of 1 equipment, LPAs obverved play structure was severely damage and Director admitted that the play structure was not fase for usage, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation during physical plant inspection, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. A slide in the outdoor play structure has a 3 inch crack that could be hazardous to children that use the slide.

Based on record review of staff files the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Two of the ten staff files reviewed did not have proof of immunization against pertussis and measles, and One of the Ten files did not have proof of measles immunization available to review.
