Based on observations, interviews, and review of Guardian the facility did not ensure background clearance was obtained for 2 adults (staff#3 & #4), who were in care and supervison of children which poses an immediate health, safety or personal rights risk to persons in care. 8 Director provided to LPAs with a declaration regarding both adutls will not return to the facility until have their criminal record clearance and been associate to the facility.

Based on observations, during this visit, LPAs Miranda& Hernandez, reviewed files of Staff#1, #2 and #3 and observed, that facility was unable to produce proof of teachers qualification for them. This poses a potential health and safety risk to children in care. Development Permit issued by California Commission on Teacher Credentialing.
Based on observations, during this visit, LPAs Miranda& Hernandez observed that facility has a cement pole, located at the outdoor space in front of the classroom#9. This poses a potential health and safety risk to children in care. 8 During this visit, Director placed a tape on the top of the wire, however Director will submit photos via email to LPA Miranda no later than 01/06/2023 with the final plan of correction.

Documents will be mail or LPAs observed that facility sketch does not match with physical plant. 8 drop off at the El Segundo Regional Office to LPA Miranda no later than 01/09/2023.
