Based on interviews and record review, the licensee did not comply with the section cited above. In the course of the investigation, 1 Site Supervisor agrees to submit a written plan/statement as to how they will assure compliance with the cited section. Written plan/statement to be submitted to the Riverside Child Care Regional Office by 1/30/2026 it was determined that some parent/legal guardians had been informed that the facility closes at 5:30pm and/or earlier than 6:00pm, which differs from the parent handbook and facility license, which state 6:00pm. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. In reviewing staff records, staff #10 was hired with a TB test/Health Screening that had been conducted over a year ago. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #4, #5, #6, #7, #8, #11, and #12, were missing an Acknowledgement of Receipt of Licensing Reports (LIC9224) for a Type A citation that was issued on 8/14/2025. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review the facility has not completed the testing of their water, that is used for consumption, for lead. This poses/posed a potential health and safety or risk to persons in care.

Based on observation of the facility van used for transportation that had a magnetic sign which had the facility name and logo but not the facility license number. This poses/posed a potential health and safety or risk to persons in care.

LPA observed a staff providing care and supervision to 5 and then six infants, alone, without the help of additional staff. 1 Director agrees to submit a written plan regarding staffing placement to assure compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 8/15/2025. This poses an immediate health, safety or personal rights risk to persons in care

LPA observed an infant teacher leave the infant classroom 1 Director agrees to submit a written plan regarding staffing placement to assure compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 8/15/2025. and enter the another classroom to assist a parent dropping their infant child. In doing so, the infant teacher left four infant alone in the classroom. This poses an immediate health, safety or personal rights risk to persons in care

LPA was informed and staff file was reviewed, which confirmed that the staff that was left alone with up to six infants had not completed the required courses to 8 be left alone with infants. This poses/posed a potential health, safety or personal rights risk to persons in care.
