Based on interviews conducted by LPA and a thorough review of facility records, it has been determined that the licensee failed to comply with the regulation cited above, as detailed in LIC 9099. This poses a potential risk to the health, safety, or personal rights to children in care. 8 In addition, the Site Supervisor has implemented the assignment of an additional staff member to ensure direct and continuous supervision of children with challenging behavior.
Accountability. The licensee, whether an individual or other entity, is accountable for the general supervision of the licensed childcare center and for the establishment of policies concerning its operation. LPA 1 Director will conduct a staff meeting and instruct on chain of command and policies. (By POC date) determined that facility staff altered the food program to a child in care without prior approval. This poses a potential Health & Safety risk to children in care. 8

Staff #1 and #2 barricaded a child in an area to force him/her to sleep without giving him/her the option of a quite time activity. This poses a potential risk to the health, safety, or personal rights of children in care.
