Reporting Requirements f) As soon as possible but no later than the same business day, the licensee shall notify a child's parent or authorized representative regardless of the injuries or acts that affect that child as specified in Health and Safety Code Section 1597.467(a). POC: Director stated that a training was conducted on August 21,2026 that covered reporting requirments, safety and active supervision. LPA received meeting notes and signatures of the staff that attended the training. (1) Any injury suffered by a child in care shall be reported to that child's parent or authorized representative regardless of treatment by a medical professional. This requirements was not met as evidence by: interviews conducted revealed that the facility failed to inform child#1’s parents of the injuries to child's face. This is a requirment that if not corrected can pose a risk to the health and safety to the children in care.
C1 left the facility playground without staff knowledge and was found by the facility interior hallway that leads to the front desk unsupervised by the front desk staff.This is a deficienyc that if not corrected poses an immediate risk to the health, safety, or personal rights of children in care.
