Per director, staff are no longer additional items like crib sheets. Staff were written up and received training on safe sleep regultations. LPA received documentation on 09/30/24 of staff write ups and proof of training. LPA observed through video footage dated 09/20/24 and S1 acknowledgment that staff were observed to place a crib sheet on the side of the crib rail of a napping infant. LPA also observed staff swaddled an infant, placed the infant on a boppy pillow, and covered the infant with a crib sheet. 8 Request Denied

Personal Rights: The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to... 1 Director will train staff members on how to properly pick up an infant from cribs after napping. Director will provide a sign in sheet of staff training. This requirement was not met as evidence by: Based on LPAs observation of vidoe footage obtained by LAPD, it was observed that S1 pressed on an infants pelvis area, squeezed infants cheeks and abruptly lifted the infant out of the crib. This poses an immediate health and safety risk for children in care. 8
