Based on (record review), the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. **Last drill conducted was documented on 3/28/2025.

Facility will make the outlet inaccessible to children in care immediately. By 02/09/2026 Director will send a written plan of remediation, ensure outlets are re-tested and submit test results. Per lead testing results, Outlet B inside the Caterpillars classroom exceeded 5ppb.

(1) External Water Sampler Self-Certification Form (LIC 9275) for certified external water samplers. (2) Facility Sketch (LIC 999) fully labeled with the locations of all water outlets, including outlets that will not be sampled. Based on interviews, the licensee did not comply with the section cited above when C1's glasses were broken by C2 and staff did not witness incident which poses an potential risk to the health, safety or personal rights of children in care.

Based on records reviewed and interviews conducted, it was determined that the preschool is not a Spanish immersion school as the Parent handbook describes. This poses a potential risk to the health, safety, and personal rights of children in care.
