Director to have S1 fingerprinted and associated and submit a written plan of correction to LPA by 10/16/2025. During today's inspection, LPA observed staff S1 showing an “In Process” background clearance. Director confirmed that S1 has been employed at the facility since 08/18/2025. No proof of criminal record transfer request or clearance before 08/18/2025 was provided. This poses a potential risk to the health, safety and personal rights of children in care.

Based on record review, LPA observed that child’s authorized representative was not informed of bump (insect bite) on the child that child received at the facility in a timely manner. This poses a potential risk to the health, safety and personal rights of children in care. 8 Deficiency Dismissed

Based on director and staff interviews regarding insect bites, they stated that there were several children, approximately 3 or 4 children, that were observed with bug bites during the summer months. Director failed to not submit an incident report to the Child Care Licensing Department. This poses a potential risk to the health, safety and personal rights of children in care.
