Based on observation, the licensee did not comply with the section cited above in LPA observed chipped paint on classroom walls, chipped/peeling paint on a table, carpet transition pieces are spreading apart causing a possible tripping hazard; which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in LPA observed boards on building have damage, plastic house and kitchen are not in good repair, spider webs, painting of city scene along fence is peeling and falling off, the side of the building that is inaccesible to children is piled with wood, paint containers, other debri that could be fire hazard and needs to be cleaned out. These items which pose a potential health, safety or personal rights risk to persons in care.
