Director will ensure that Infant sleep logs are to be completed every 15 minutes in real time as infants sleep. LPA observed C1 sleeping in the crib on their stomach at 1:13pm. LPA reviewed infant sleep log. Sleep log was completed by staff and signed for times 12:45p, 1:00p, 1:15p, 1:30p, and 1:45p at 1:13p.

Provider will sign LIC 9227 form no later 24 hours for child’s file. Director will provide signed LIC 9227 form to LPA for review. LPA completed a file review for infant child C1. LPA observed file to be missing LIC 9227.
