Based on the investigation, it was determined that on or about 9/29/22, during nap time, there was lack of visual supervision of children resulting in the inappropriate contact between child #1 and child #2. 1 Director shall provide LPA with a plan for care and supervision during nap time. The plan should entail a process to ensure children are observed at all times and provide training to all staff to prevent similar incidents reocurring in the future. The plan must be submitted to the depament no later than 02/15/2023. This is a Type A violation which poses an immediate health, safety or personal rights risk to persons in care.
