101238 (a) The childcare center shall be clean, safe, sanitary... at all times to ensure the safety and well-being of children,(1) The licensee shall take measures to keep the center free of insects, and rodents.This requirement is not met evidenced by: Director stated, one staff member will attend Integrated Pest Management Training, certificate will be provided to LPA by POC date. Proffessional pest control assesment will be conducted and report will be provided to LPA. Further,Pest management will be conducted if needed. Per Director roaches were observed in the kitchen area and classroom nearby, which was confirmed by two staff members, which posses potential health and safety risk to children in care. Additionally, LPA observed two dead roaches in the kitchen cabinet and one in the cabinet under the sink located in the 2A classroom. Cocroach traps and baking soda observed in the cabinets. 8

Based on observations, records review and interview , the licensee did not comply with the section cited above. 6 out 9 staff files were missing Mandated Reporter Training , which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations , records review and interview , the licensee did not comply with the section cited above. Three out of nine records were missing proof of mmr and Tdp, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on five staff file review, the licensee did not comply with the section cited above in 3 out of 5 staff, Staff#1, staff#2, and staff#3 did not have Health screening form on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on 10 children files review, the licensee did not comply with the section cited above in 2 out of 10 children did not have Physical Report on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on sign in/out sheet review, the licensee did not comply with the section cited above, during this inspection 74 children were present and 63 children were signed in which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on facility file review and director statement the licensee did not comply with the section cited above and has not completed water lead testing yet which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on four staff file review the licensee did not comply with the section cited above in 3 out of 4 staff reviewed files, staff#2, staff#3, and staff#4 were missing required mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on four staff file review, the licensee did not comply with the section cited above in 3 out of 4 staff reviewed files, staff#2, staff#3 and staff#4 were missing TB test clearance on their files which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on 10 children file review, the licensee did not comply with the section cited above in 2 out of 10 children reviewed files, child #3 and child #4 were missing physician reports on their files which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on facility 02/03/2023 sign in/out sheet review, the licensee did not comply with the section cited above. During this inspection 71 children were present and only 62 children were signed in which poses/posed a potential health, safety or personal rights risk to persons in care

Staff do not keep the facility free from infestation.
