LPA observed that sufficient staff were not present for the number of children in the TK/ Kindergarten classroom on several days in the month of August.

the Facility was operating out of ratio when one partially qualified teacher opened the Facility on the 1 Jordan Armstrong, Director, agreed to submit a written Plan of Correction (POC) by Tuesday September 3, 2024. The Plan of Correction shall include the Facility protocol for ensuring that a director or fully qualified teacher shall be present to open or close the Facility at all times. following dates: 05/14/24, 05/16/24, 05/21/24, 05/23/24 and was the only staff present with anywhere from one to six children present during the early morning session. This presents a potential risk to the health, safety, or personal rights of children in care.
the Notice of Site Visit from the Department's last visit to the Facility on June 4, 2024 was not posted upon LPA's arrival to the Facility. 8 Deficiency cleared.

Based on observation, there was not a cover on the electrical outlet near the entrance door that has the stairs. This poses a safety hazard to children in care.
Training of staff is to be completed and a outline of the training and a sign in sheet will be submitted to the department before the plan of correction date. Staff was not watching the children as it was a maintenance worker from the school district who saw the children and reported it to the Director.

Letter should indicate all documents to be obtained.
The Licensee will transfer/complete fingerprint clearances for those not associated to the facility. Any employees who do not have clearances should not be present in the facility until they have been obtained. The Licensee did not complete criminal record clearances/transfer prior to initial presence in the facility for seven staff members which poses an immediate risk to the health, safety, and personal rights of children in care. 8 Civil penalty assessed.

Area Manager will submit proof of Site Director qualifications and be sure to include education requirements, proof of experience from previous employers, MMR vaccination by 2/3/2023 Area Manager indicated Sarah Hinkle on LIC200A, however did not submit qualifications for review which poses a potential threat to the health, safety, and personal rights of children in care. 8 All required qualifications paperwork should be submitted for review.
Area Manager, Anjelica Del Toro, will submit proof of education/transcripts, Mandated Reporter training, immunizations (MMR, Tdap, flu), and required experience for review to determine qualifications of staff. The licensee failed to maintain personnel records as many required documents were missing which poses a potential threat to the health, safety, and personal rights of children in care. 8 Staff are additionally missing proof of physicians report and negative TB test, which should also be submitted. All documents should be submitted by 2/3/2023 for all staff present during 2022-2023 school year.
