Based on observation, interview and record review, the licensee did not comply with the section cited above for one staff, S2 which poses a potential health, safety or personal rights risk to persons in care. Staff was observed to be missing proof of measles immunization, proof of negative tuberculosis test and Health Screening Report (LIC 503)

LPA observed that Staff, S1 is cleared and associated with the company but not associated with this facility location.

Based on observation, interview and record review, the licensee did not comply with the section cited above for one child, C6 which poses a potential health, safety or personal rights risk to persons in care. LPA observed child, C6 has an Epinephrine Autoinjector that expired in October 2022 and has not been returned or renewed by facility.

After water lead testing was completed, the facility had one faucet (F) located in the indoor preschool classroom with an Action Level Exceedance of 16, which poses a potential risk to the health, safety, and personal rights of children in care. The Site Director covered and taped the ALE bubbler and posted signage to ensure it is not used. Site Director will also submit a written letter stating that bubbler "F" will remain out-of-use and will ensure that it is not used for food preparation or children's drinking water. Director submitted letter of understanding while LPAs were at facility.
