On 01/19/2022 The licensee did not ensure 1 The director agrees to create and provide a new letter to all staff and parents advising that all staff and children over the age of 2 years old will wear a protective face covering. Staff will sign off on the letter, and a copy will be maintained in their files. Director will send a copy of the letter to LPA by 5:00pm, on 02/03/2022. The personal rights of persons in care for safe & healthful accommodations, where LPAs observed 38 children, not wearing face coverings or had an individual mask exception as required by the CA Dept. of Public Health Guidance issued on July 28, 2021, which is a potential health & safety risk to children in care.

Based on information gathered through interviews conducted, the licensee failed to report Unusual Incidents to Community Care Licensing upon receipt and knowledge that staff and children tested positive for COVID-19, which poses and potential risk to the health & safety of the children in Care.

Based on the facility sign-in and sign-out sheets reviewed for 02/02/2022 it was determined that 12 children had not signed in after a census was conducted by LPAs observed 147 children, which poses an potential risk to the health and safety of the children in care. 8 The written statement and supportive documents will be submitted to community care licensing by 02/07/2022.
