Based on observtion, the facility playground has holes on its artificial turf which poses a potential health, safety and personal rights risk to persons in care. 1 Per Director, they will block access tobig slides where damaged turf is and she will contact Administrator Antony Harshi to repair damaged turf. Director is to contact LPA Duran once turf is repaired no later than 08/21/2026.
Based on LPA record review, the Licensee did not comply with the above regulations. C2 and C3 did not have prescriptions medications with label directions at time of inspection. This poses a potential health, safety, and personal rights risk to children in care.
