Based on UIR review by LPA Calderon injury occurred 5/27/26; Licensing was called by former Director Dena Hardin on 5/29/26 to report injury that required medical professional, facility did not met required reporting requirement in a timely manner to report 8 Per RANASINGHE, D. they will do a plan on what was trained regarding Reporting Reporting and who conducted the training and submit to LPA Calderon.
Based on record review and observation, the center has not conducted the required lead testing as of 05/30/23, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
