On 08/06/2026, a child was left unattended outside the classroom, which posed an immediate risk to the health, safety, and personal rights of children in care. 8 Upon receipt, the Director shall post this report and provide copies of this licensing report to parents/guardians of all children currently in care. The Director shall also provide copies to parents/guardians of children newly enrolled in the facility for the next 12 months. The LIC 9224 Acknowledgement of Receipt must be signed by parents/guardians and kept in each child's file. In accordance with AB 633, parents must be provided with this report, which contains a Type A deficiency, for the next 12 months, and a copy of the signed acknowledgment form must be maintained in each child's file.
