Responsibility for Providing Care and Supervision for Infants - (a) In addition to Section 101229, the following shall apply: (1)Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. 1 Center Director must provide to the LPA proof of training to staff regarding supervision to children in care. Proof was received during LPA annual visit on 10/20/2025. Based on observation, interview, record review, the licensee did not comply with the section cited above in that a child was left unsupervised, which poses an potential health, safety or personal rights risk to children in care. 8

101238(a) Buildings and Grounds: The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. Licensee will either replace the faucets with new ones and have them tested again or disconnect and place the faucets out of service permanently. Either the new test result or proof of permanent removal of the faucets will be emailed to LPA Woods no later than the due date of September 30, 2023. Sample ID II - Sink Faucet ,Sample ID XX , and Sample ID XX30 had a Lead Action Level of Exceedance Response of more than 5.5ppb.
