101700.3 (b)(1): A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidence by: 1 Facility agrees to implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Based on records review of required lead testing, the facility had lead values of 5.5 or above on water Outlet A, level 11.0 ppb exceedance located at the infant kitchen sink. This is a potential health and safety risk to persons in care. 8 The Facility agrees to notify CDSS with completion within 30 days.

An infant was found by a parent outside of 1 The director stated the staff have been retrained and the staff member who opened the door is no longer employed at the school. The director will submit an agenda from training with all staff signatures to verify training. andrea.taylor@dss.ca.gov the classroom on the sidewalk. A teacheropened the door to the parking lot and a child was able to exit the room without the teacher seeing the child exit. This poses an immediate risk to the Health and Safety of the children in care. Please note that a $500.00 civil penalty is also being assessed.
