Based on observation and interview the faciility did not comply with the section cited above in the carbon monoxide was not working properly and the assistant director ordered one to have it replaced. This poses an immediate health, safety or personal rights risk to persons in care.

Based on observation & interview the licensee did not comply with the section cited above in the kitchen area had two panels missing from the ceiling and there was a hole in the middle of the kitchen visible to wires and plumbing fixtures above the ceiling. Ms. Avila stated the owner has it that way to ventilate the air because of COVID. which poses/posed a potential health, safety or personal rights risk to persons in care.
