All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f) 1 Director stated that she will ensure that all staff members are associated to the facility prior to them working at the facility. Director will submit a written plan of correction to Licensing no later This requirement is not met as evidence by S1 and S2 was working at the faciltiy prior to their background clearance being associated to the facility. S1 is no longer an employee at the facility and S2 worked at the faciltiy for a couple months prior to being associated to the facility. Civil penalty of $1000 is being assessed today. This poses a potential health and safety risk to clients in care. 8 than 10/26/2022.

Prior to employment, an infant care teacher shall have completed, with passing grades, at least three postsecondary semesters or equivalent quarter units in early childhood education or child development, and three postsecondary semester or equivalent quarter units related to the care of infants, at an accredited or approved college or university. Director stated that the facility will try to hire a temporary toddler teacher today and if at the end of the day they are unable to hire a temporary staff then the facility will have to stop providing care for the two toddler age children. This requirement is not met as evidence by LPA Siao's observation. Licensee Anita Okolo was observed to be supervising the three toddlers in care today and she is in the process of completing the infant course. This poses a potential health and safety risk to clients in care. 8 Director will also submit an exception request to the department. A written plan of correction will be submitted to Licensing no later than 10/26/2022.

Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below: (1) A teacher shall have completed, with passing grades, at least six postsecondary semester or equivalent quarter units of the education requirement specified in (c)(1) below, or shall have obtained a Child Development Assistance Permit issued by the California Commission on Teacher Credentialing. 1 Director stated that she will make sure that another teacher stays in the Bigger Dipper two year old class with Maria Montenegro and that teacher aide are always with another teacher. Director will This requirement is not met as the two year old staff Maria Montenegro and teacher aide Yensenia Robles does not have the required units to be teachers and they were observed alone superiving the children during today's inspection. This poses a potential health and safety risk to clients in care. have a meeting with staff members regarding the regulation for teacher and teacher aide. A staff's signed in sheet and training agenda will be submitted to Licensing no later than 09/19/2022.
