This requirement was not met by LPA's observation and records review, Staff #1. Does not have Mandated Reporter Training Exp: 5/28/25, which poses a potential health and safety or personal rights risk to persons in care. 8

Based on observation the school aged children were sitting in their area in the dark because the main lights were off due to preschool children napping. There were no other lighting options observed during the inspection which poses a potential health, safety, or personal rights risk to the persons in care.

(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful, and comfortable accommodations, furnishings, and equipment to meet his/her needs. (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature including but not limited to interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. S #1 employment has been terminated and a training was conducted on proper way to redirect he children. Also if the staff need assistant in the classroom they can call out to the director for help. per director the facilty will submit proof of training conducted by Licensee.by

Based on Interviews for complaint investigation, Staff 1 yelled at Child 1, day care child while in care which poses an Immediate Health, Safety, or Personal Rights risk for the persons in care. 8
