Based on observation, interview, and record review, staff (S-1) who was present at the facility has cleared fingeprints, but fingerprints are not associated to facility. This poses a potential health and safety risk to children in care.

Based on tour observation indoor and outdoor in the Infant Center, LPA observed broken toys and rusted swings outdoors that were not placed inaccessible to children in care. LPA also observed uneven walking surfaces and tree roots making it a tripping hazard to children in care. LPA also observed an old wood shade structure that is a health and safety concern. LPA observed rip mattresses in all infant cribs, two broken cribs, broken blinds, unsanitary carpet in crib area and in infant indoor playground structure, stains in walls & chip wall paint. This poses a potential risk to the health and safety of children in care.
