This requirement was not met by: 1 Center Director states S1 and S2 were provided disciplinary action, one on one training for name to face and transistionOn 6/17/25 Training conducted for all staff regarding name to face and supervision. Name to face policy was update for specific verbage. Based on interviews the faciliy did not ensure the supervision during transition from playground to classroom, leaving C1 unsupervised from approximately 2-5 mins. Which posed a potential Safety rights to persons in care. 8

One of the staff's online mandated child abuse training completion did not show her name. This poses/posed a potential health, safety or personal rights risk to persons in care.
