Limitations on Capacity
Facility will conduct a training covering rules around comingle and supervision. Plan of Correction it to be submiteed by 5/8/2025 and will follow-up after training to submit proof of training. LPAs observed two preschoolers comingled with school age children which poses an immediate risk to the Health, Safety or Personal Rights to children in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Reporting Requirements: a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event 1 Prior to the initial complaint investigation the facility submitted a UIR to ccld for the incident named in the complaint. POC cleared by visit. (C)Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. Based on interviews and observations staff didn't report an incident to a parent on pick up which is a potential personal rights risk to children in care.
