child (C-1) was found unsupervised in the parking lot playground by C2 grandparent. This is based on a self report made by facility and interviews conducted with staff. This poses an immediate risk to the health and safety of the children in care. 8 The Director will notify CCLD upon completion of the task no later than 10/18/23, close of business via email. Director is to submit proof of plan of additional supervision measures.
