Based on observation, interview and record review, the licensee did not comply with the section cited above. At approximately 8:55 AM, LPA observed child #1 (C1) on the play yard. LPA observed C1 did not look to be two years of age. LPA asked Im Kyoung Oh the age of C1 and Im Kyoung Oh stated (two (2) years old.) During children file reviews at 9:20 AM, records show C1 is 22 months old. The department considers infants to be 0 to 24 months of age. The facility is licensed for children 2 to six years of age. Based on documentation, C1 enrolled on 7/6/2026 which poses an immediate health, safety or personal rights risk to persons in care.
Based on interview, the licensee did not comply with the section cited above. LPA verified during interview that the lead testing was NOT completed in accordance to the Written Directives which poses/posed a potential health, safety or personal rights risk to persons in care.
