Director states she will review proposals from plumbing companies on how to repair Drinking Fountain C by the POC date. Director understands that if Drinking Fountain C is replaced, it will need to be retested for PPB levels in order to fulfill correction. Director will inform LPA when correction is On 08/18/25 licensee failed to maintain a lead value at or below the Action Level for water lead testing resulting with values of 5.5 ppb or greater for Drinking Fountain C. Water testing results identified with Action Level Exceedance as defined in WD section 101700.3. are not deemed safe to drink. 8 completed. Licensee states will send informative letter via Brightwheel app to parents of children

Based on interview conducted, Director states she has not scheduled the center's drinking water tested for lead contamination levels which poses an potential health, safety, and/or personal rights risk to persons in care.

LPA received corroborated disclosure that a staffmember yelled at children in a loud harsh tone to get them to follow instruction. This is an immediate risk to children in care.

Staff #4 supervised children alone for approximately 1 month without having proof of ECE units 1 Director Lozano indicated that staff #4 no longer supervisies the classroom and works in an Aide role under a teacher until she completes education requirements. LPA observed this to be true during inspection. LPA also received disclosure that fingerprint cleared Church staff provided childcare on at least one occasion for approximately 15-20 minutes during a staff meeting at the facility. This is an immediate risk to children in care.
