(a) A licensee shall not operate a childcare center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met as evidence by: The Director agrees to submit a statment indicating that the center will track the transition ages of their children so that they can move into their appropriate classrooms. Based on interviews and records reviewed, the licensee did not comply with the section cited above. The facility is commingling infants and preschool children to meet ratios in the morning. This poses a potential health, safety, or personal rights risk to children in care. 8

(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. 1 Director stated that they have impleted all staff working in in the infant room will now have a indiviudalized lab coat that they will wear over their clothing and at the end the day they will leave them in the facility to be washed and dryed. Director also stated Assistant Director will be conducting a health check (smell) of the employees and their belongs pior to entering the infant room. Director also stated staff will leave their personal belongs in the staff break room. Director will submit a written statement by 1/10/2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in one out of 24 records observed that the parent or authorized representative has not signed in/out child. LPA observed a record that C1 is also on a subsidized care program and dates of attendance were entered for eight days in June that the parent or Authorized representative had not signed in the child for on the day care sign in sheet. This poses a potential health, safety or personal rights risk to persons in care.

observation. While taking a census LPA observed Child #2 asleep in a crib while wrapped up in a blanket. 1 Director immediately unwrapped child #2 from the blanket LPA provided Director with paperwork regarding Safe Sleep regulations in Child Care Centers. Director stated she will provide training to staff on Safe Sleep regulations and provide Community Care Licensing with a copy of the training itinerary This is a potential risk to the health, safety or personal rights of children in care. 8 and a copy of the sign in form showing what staff attended by 11/4/22.
