Based on parent interviews conducted. Parent stated the following: they observed staff chitchatting with each other; their child received unexplained injury and staff did not know what happened; their child has fallen on the playground multiple times. This poses an immediate risk to the health, safety, of children in care.

The director agreed to send a statement reviewing the importance of supervising children at all times with staff with their names and signatures acknowledging this section was reviewed with them. The statement will be sent to LPA's email address by the due date of 6/23/2026. Self reported incident and LPA's interviews today with 3 staff that 3 years old, C1 was left unattended in the restroom of the classroom unnoticed for approximately 8 minutes when all the staff and children were on the playground. C1's representative found C1. This is an immediate risk to the health and safety of children in care.

101223(a)(1) Personal Rights: The licensee shall ensure that each child is accorded the following personal rights: To be accorded dignity in his/her personal relationships with staff and other persons. This requirement is not met as evidence by: 1 Facility director stated she will do an all staff training on Personal Rights on 1/14/26. Director will also focuss on classrooms 5 and 6 and do Individual Observations on staff and provide indivual support to staff. Director will email LPA Meeting Agenda and sign in sheet. based on staff interview, S1 threatened to throw C1’s shoes in the trash if C1 did not put it on and S4 called C2 a “ding-dong” which was not a good thing. This pose a potential danger to the health of the children in care. 8
